Straight answers, including when we’re not the right fit. Concierge & Functional Medicine FAQ
73 questions, from memberships and insurance to every service we offer.
The Basics
What is functional medicine?
Functional medicine looks for the root cause of a problem instead of chasing symptoms. That means an in-depth look at your history, diet, lifestyle and environment, plus the right lab testing. There are no cookie-cutter plans. Each person’s pathway to feeling well is different.
Is Dr. Cas a “real” doctor?
Yes. Dr. Castel Santana is an MD and a board-certified family physician who completed residency at the University of Maryland. He is also a Certified Functional Medicine Practitioner. Prescriptions are part of his toolkit when they’re warranted. He just evaluates the other effective options before reaching for the Rx pad.
What does a membership include?
Every tier includes an initial visit, full functional lab panels with lab-review visits, a yearly exam, quarterly sick visits, nutrigenomic/methylation genetic testing and a discount on other services. Higher tiers add IVs, more testing, imaging and physician access.
What labs do you use?
Specialty testing runs through Vibrant America, a cash-pay lab known for advanced functional testing. Routine panels can be drawn in the clinic.
Before You Commit
Why is functional testing cash-pay?
Insurance generally pays for a test only when a diagnosis code justifies it. That means many useful tests can’t be ordered until a disease has already shown up. Paying cash means Dr. Cas can order what your situation calls for, you see the price up front, and results come back faster.
Do I have to stop seeing my other doctors?
No. Dr. Cas can be your primary care physician, or he can work alongside the specialists you already see.
Isn’t this expensive?
Every membership is priced below what the same visits and labs cost à la carte, and the value tables on the memberships page show every line. HSA and FSA funds may be used for eligible services.
How long until I see results?
It varies. Many patients notice changes in energy, sleep and mood within weeks of building their foundations. Gut restoration and detox work can take up to six months. Retesting shows what’s actually changing.
Who Evoke 5 Isn’t for
Who isn’t a good fit?
Evoke 5 isn’t the right choice if you want a quick prescription for each symptom and nothing else, or if you aren’t ready to change your sleep, food and daily habits. The patients who do best are curious about their data, willing to follow a plan, and OK hearing “not yet” when a treatment isn’t right for them.
Is Evoke 5 for emergencies?
No. For a medical emergency, call 911 or go to the nearest emergency room.
Memberships & Pricing
Learn more about Memberships & PricingIs a membership the same as insurance?
No. A membership covers the visits, labs and services listed in your tier, priced below their à la carte cost. It is not health insurance.
Can I use HSA or FSA funds?
HSA/FSA funds may be used for eligible memberships and services. Confirm eligibility with your plan administrator.
Are labs included?
Every tier includes full functional medicine lab panels: two a year with Evoke Well, three with Evoke Optimize and four with Evoke VIP. Other specialty labs are billed separately at transparent cash prices through Vibrant America.
What if I don’t use everything?
Services must be used within the membership year, and membership fees are non-refundable.
When does Evoke VIP open?
Soon. Join the waitlist through the consult form and you’ll hear first when enrollment opens.
Concierge Medicine & Primary Care
Learn more about Concierge Medicine & Primary CareWhat is concierge medicine?
Primary care paid through a membership instead of billed visit by visit through insurance. A smaller patient load buys longer visits, real follow-up and a doctor who knows your history. At Evoke 5, the membership also includes functional lab panels and genetic testing.
Can Evoke 5 be my primary care doctor?
Yes. Members get a yearly exam, sick visits throughout the year, lab panels with review visits, and a care team that already knows them. Dr. Cas can also coordinate with your specialists.
Do I still need health insurance?
Yes. A membership is not insurance. It covers the visits, labs and services in your tier. Your consult is a good time to talk through how it fits with your coverage.
Who can use telehealth with Evoke 5?
Telehealth is available for consults, lab reviews and follow-ups. Licensing rules decide where you need to be during a visit, so the team confirms what's possible for your location when you book.
Can I do everything by telehealth?
No. Exams, injections, IVs, plasma exchange and aesthetic treatments happen in the McKinney clinic.
Functional Medicine & Testing
Learn more about Functional Medicine & TestingWhat does a functional medicine doctor do differently?
Looks for the root cause instead of treating each symptom on its own. That means a deep look at your history, diet, lifestyle and environment, plus lab testing that goes well beyond a standard panel.
Is Dr. Cas a medical doctor?
Yes. Dr. Castel Santana is an MD and a board-certified family physician who completed residency at the University of Maryland, and a Certified Functional Medicine Practitioner. Prescriptions are part of his toolkit when they're warranted.
Which test should I start with?
Most people start with the full functional lab panel and genetic testing, both included in every membership. Gut, toxin and oxidative stress testing get added when your history or first results point that way.
Why don't you bill insurance for labs?
Insurance generally pays for a test only when a diagnosis code justifies it. Vibrant America is cash-pay, so Dr. Cas can order what your situation calls for, and you see the price up front.
How long do results take?
Routine labs often come back within 10 to 14 days. A full specialty baseline takes about four weeks.
How are samples collected?
Depending on the test, from blood, urine, stool, saliva or a swab, in the clinic or at home. You'll get clear instructions either way.
How is a food sensitivity test different from an allergy test?
It looks at immune reactivity to common foods plus additives like preservatives, artificial sweeteners, emulsifiers, dyes and pesticides. It helps explain symptoms standard allergy testing misses.
Do I need gut symptoms to benefit?
No. Fatigue, brain fog, skin, joint, weight and hormone problems can have a strong gut contribution even when digestion seems fine.
Will I just be told to take probiotics?
No. The bigger levers are usually removing what irritates the gut, supporting digestion, feeding good bacteria with diverse fiber and repairing the gut lining.
Who should consider toxin testing?
Anyone living or working in an older or water-damaged building, with chronic fatigue, brain fog or "mystery" symptoms, or with autoimmune, hormone or gut problems that aren't resolving.
What does the panel measure?
20 heavy metals (including lead, mercury, arsenic, cadmium and aluminum), 29 mycotoxins from mold, and 38 environmental chemicals like glyphosate, phthalates and bisphenols.
Do you start a detox right away?
No. Removing exposure and opening the body's elimination pathways come first. Mobilizing stored toxins before your body can clear them can cause reactions.
Can't I just take a multivitamin?
A multivitamin can't tell you what you're low in, and some nutrients cause harm in excess. Your body may also not be absorbing what it gets. Testing answers both.
Why would my levels be low if I eat well?
Gut problems, medications, stress, genetics and alcohol all affect absorption. That's why this test is often paired with gut and genetic testing.
Does an MTHFR mutation matter?
It depends. MTHFR helps make the active form of folate. Whether a variant matters for you depends on your nutrients, homocysteine, gut health and symptoms. A genetic result alone doesn't tell the whole story.
What is the Dr. Cas Medical Avatar?
An interactive educational tool in your patient portal that explains your labs, genetics and the reasons behind your recommendations. Genetic testing unlocks it.
Will genetic testing tell me if I'll get a disease?
No. Nutrigenomic results guide diet, supplement and lifestyle choices. They don't diagnose disease.
What is oxidative stress?
Making energy creates by-products often called free radicals. Oxidative stress is what happens when they overwhelm your antioxidant defenses and start damaging proteins, fats and DNA.
Shouldn't I just take antioxidants?
Not blindly. High doses can blunt helpful signaling. Evoke 5 measures first, lowers the biggest sources, then rebuilds your body's own defenses, especially glutathione.
Is this a cure for autism?
No. It doesn't replace an autism diagnosis or the therapies that support it. It asks which modifiable stressors, like methylation bottlenecks, toxin load or dietary inflammation, might be adding to symptoms like poor sleep, GI trouble or irritability in this child.
Who is the program for?
Children and adolescents with a confirmed or strongly suspected diagnosis whose families want a time-bounded workup instead of an open-ended supplement list.
How is progress measured?
With a short, family-chosen scorecard, such as sleep, stool pattern, irritability, shared attention and energy.
What does the program cost?
Program details and investment are reviewed at the consult, before anyone enrolls.
Hormone & Wellness Therapies
Learn more about Hormone & Wellness TherapiesDo I need labs before starting hormones, peptides or IVs?
Yes. Hormone therapy starts with blood work, and every IV is clinician-directed with any required labs confirmed first. Therapies are layered onto a plan, never sold off a menu.
Do members get a discount on these therapies?
Yes. 5% off with Evoke Well, 10% with Evoke Optimize and 15% with Evoke VIP. Optimize also includes a Myers' Cocktail IV every three months, and VIP includes one monthly.
Are these therapies safe?
They're medical treatments with risks as well as benefits. Each one starts with an evaluation, uses ongoing lab monitoring and is adjusted by a physician-led team. Not everyone is a candidate.
What are hormone pellets?
Bioidentical hormones compounded from soy and yam, about the size of a grain of rice. They're placed under the skin in a quick in-office procedure and release small, steady amounts of hormone.
How long do pellets last?
Up to about three months for women and up to about six months for men, adjusted to your labs and symptoms.
Is hormone therapy only for older adults?
No. Levels typically decline around age 50, but low levels can happen much earlier. Evoke 5 has treated men in their 20s with low testosterone. Testing shows where you stand.
Are peptides the same as hormones?
No, though some influence hormone pathways. Many work by signaling your body to make more of what it already produces, such as growth hormone, rather than replacing a hormone directly.
How are peptides given?
Usually by injection, which tends to work best; some are oral. You'll be shown how to use them safely.
Do I need labs before an IV?
Every infusion is clinician-directed, with the reason and any required labs confirmed first. Some, like iron, always require labs.
Is IV therapy safe for everyone?
Most people handle extra fluid well. Important exceptions include heart failure and kidney failure, so your history is reviewed first.
Regenerative & Longevity
Learn more about Regenerative & LongevityDo I have to decide on a treatment before the consult?
No. The consult is where that decision gets made. Sometimes the answer is something simpler, or nothing yet.
Who isn't a good candidate?
It depends on the therapy. Common reasons to wait include active infection, pregnancy or breastfeeding, certain cancer histories, clotting problems and unstable medical conditions.
How much do regenerative treatments cost?
Procedures are planned after consultation and priced for your plan. Call (972) 468-9766 for pricing. Members get 5–15% off depending on their tier.
Which conditions respond best?
Knee osteoarthritis has the strongest evidence. A 2013 randomized trial in the Annals of Family Medicine found dextrose prolotherapy outperformed saline injections and home exercise at 52 weeks. Tendon pain and unstable joints are also treated, with more mixed evidence.
Does it hurt, and what's recovery like?
Expect an ache for one to three days; usual activity returns in a day or two. Improvement builds over weeks, so it isn't the instant quieting of cortisone.
Who shouldn't get injections?
Allergy to dextrose or lidocaine, infected skin at the site and fever are reasons not to inject. Joint replacements, inflammatory arthritis and blood thinners need careful judgment.
What's in the infusion?
Two prepared umbilical-cord tissue matrix products, combined in a separate saline bag. Your physician reviews the formula, dose and timing at the consult.
Who should not have it?
It's held or declined for a history of cancer or bone marrow dysplasia, prior clotting events, immune suppression or chemotherapy, infection or fever, pregnancy or breastfeeding, sickle cell anemia, hepatitis B, advanced kidney or liver disease, or certain abnormal blood counts.
How is it different from PRP?
PRP depends on what your own blood supplies that day. These products are prepared to a consistent specification, with no blood draw for the product itself.
Why albumin instead of donor plasma?
Donor plasma raises the risk of transfusion reactions and infection, and of moving someone else's inflammatory load into your body. The goal is removal, and albumin-based exchange has a long safety record.
What does it feel like?
It's a closed circuit. Your blood never contacts anyone else's. Tingling, fatigue, lightheadedness or bruising at the IV site can happen and are monitored in the chair.
Will one session clear all my toxins?
No, and be wary of anyone who says so. A spaced series changes the pattern, and source control still comes first.
Who should wait?
Anyone unstable, acutely infected, severely anemic, allergic to albumin, pregnant, or with active cancer.
Medical Aesthetics
Learn more about Medical AestheticsWhy get aesthetic treatments at a functional medicine practice?
Because skin reflects what's happening underneath. Hormones, gut inflammation, protein intake, sleep and sun all affect how well new collagen holds, so results last longer when those are addressed too.
Can I get treated at my first visit?
Often, yes. If your skin needs barrier repair, a hormone review or a series plan first, that becomes the recommendation, because it protects your result.
Which neuromodulator do you use?
Jeuveau®, at $13 per unit. The number of units is set at your consultation.
What can neuromodulators treat?
Frown lines, forehead lines, crow's feet, a lip flip, jaw tension and excessive sweating. A lighter technique can also reduce oil production for some patients.
How long do results last?
About three to four months. With repeat treatments, many patients notice their resting face looks smoother too.
PRP or exosomes, which is better?
Different tools. PRP comes from your own blood the day of treatment and has the longest track record. Exosome preparations come from screened human birth tissue, so they don't depend on your platelets that morning. Many plans alternate them.
What is recovery like?
Redness for one to three days. Use bland barrier care and skip retinoids and unprotected sun until the skin has closed.
